Radiologists' Qualifications

All Telerays doctors must complete our credentialing process.  This includes providing copies of the following documents:

• Copies of each current state medical license showing expiration dates
• Copy of curriculum vitae
• Proof of current medical malpractice insurance showing the individual radiologist as the named insured and showing the policy expiration date
• References
• Copy of certificate from the American Board of Radiology
• Listing of category 1 CME completed in the past year
• Copy of sample radiology report (excluding patient specific information in order to respect HIPAA)

Each radiologist must complete the Telerays credentialing application which includes inquiries about the radiologist’s contact information, educational background, practice history.

Each radiologist must answer standard disclosure questions including previous licensure issues, sanctions, medical malpractice litigation and criminal history.
